Select "Description text" from Text log in ChaRM for mailform (SMARTFORMS)

Hello experts,

We have configured ChaRM and now maintaining mailforms for particular actions performed. The issue here is, when we select the text log to be sent in mailform, it selects the whole Text Log which contains "General Note" as well. But our requirement is to only pick up "Description of change" text and send it in mail.

Kindly give your suggestions as to how the text "Description of change" can be picked in SMARTFORMS.
